Fleurish is a program of South Bay Primary Medical Group, the trade name for Ciecal RC, Inc., a California corporation. Two documents, because two different things are being protected
If you become a patient and are screened, prescribed for, or treated at Fleurish, the health information created in that process is protected health information under HIPAA. That is governed by our separate Notice of Privacy Practices, which explains your HIPAA rights and how to reach us about your medical record. That notice controls for anything it covers.
This policy covers everything else: what happens when you visit fleurishweightloss.com, fill out a contact or newsletter form, or otherwise interact with us online, before you are ever a patient, and apart from your medical record if you become one.
Information we collect
Information you give us directly: your name, email address, and phone number if you use a contact or inquiry form; your email address if you sign up for the newsletter; and whatever a form on this site actually asks for, never more.
Information collected automatically: our web host and any content delivery service log standard technical data (IP address, browser type, device type, pages visited, timestamps) for security and troubleshooting.
How we use your information
To respond to your message, to send the newsletter if you signed up for it, to operate and secure the website, and to comply with the law. We do not use it to build an advertising profile of you.
How we share your information
We do not sell your personal information and do not share it for anyone else’s advertising or marketing. We share it only with service providers who help us run the website, under terms limiting their use of it, or if required by law or to protect someone’s safety. We do not use advertising or remarketing pixels of any kind on this website (Meta, Google Ads remarketing, TikTok, LinkedIn, or similar).
Cookies and other tracking technologies
The site uses necessary cookies to run and remember your choice. On a first visit, a banner offers Reject all, Accept all, and Manage choices for optional Google Analytics. The Google Analytics tag is held from loading before consent; accepting analytics allows it to load. You can reopen these choices through the Cookie settings link in the footer.
